We are pleased to welcome new patients resident within our catchment area to the Practice list. The map below shows the area we can cover.
Catchment Area
If you are within our catchment area you may register with us online. If you cannot register online you can attend a surgery and complete a paper form.
To register online please choose the form below for your nearest practice. You will need your NHS Number to register. You can obtain this from your previous GP Surgery.
If you are not able to register online you can obtain a paper version of this form from the surgery (please insert the form that is attached). You may also download this and print it off at home.
When completed, the form should be returned to our reception team. The staff will register you with the practice and you will then be able to book you an appointment.
Temporary Registration
If you become ill whilst you are away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact one of our surgeries. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
