Please print off the leaflet and hand in the completed form to your nearest surgery. This will forwarded on to our Carers Champion, who will add you to our carers register, and send out a Carers Information Pack to you in the post.
Are you a Young Carer aged 18 or under ?
Young Carers are children under 18 with caring responsibilities to a parent, sibling, relative or friend. The person they help care for may be ill, disabled, or dependant on drugs or alcohol. If you are, you can highlight yourself to us by completing the above leaflet and hand it in to your nearest surgery or even make an appointment to see a GP of your choice to discuss what help maybe accessible to you.
